If you are new to the area, new to the church or your contact information has changed, please fill out the this Form. If you prefer to print your register form, there is a link at the bottom of this page. Either drop it off at the Parish Office or email it to us at business@stpgranby.org.

Parish Registration/Census Form Frequently Asked Questions

  • They are for the Parish Office use only. Please do not write in these areas.

  • Since the merger of St. Therese's and St. Bernard's Parishes involves the consolidation of the Parish Offices, requests for information (Baptism, Communion, Confirmation, Marriage, Death) will now come into St. Therese Parish's Office. It will facilitate the search for this information to know which set of archives to look through.

  • Census/Registration forms are for recording census Information only. All Information on this form Is put into our database. Once data entry had been completed, these forms are filed for reference purposes by office personnel. If you have a question for the Parish Office, STAFF Office, or Pastor, please submit separately.

  • We need the name that appears on your birth & baptismal certificates. We do not record nicknames.

  • If you never completed a Parish Census form, we wouldn't have you in our database. Once you are an adult (21 years old and over), marry, move out of the family home, etc., you need to complete a Census form to establish yourself as a member of the Parish. Parental membership does not transfer to you once you've reach adulthood.

  • There is a spot at the end of the Town/State/Zip Code line where you can request Parish Envelopes (check yes or no), or to indicate if you will be registering for electronic cont1ibutions once you receive our welcome package with instructions.

  • Sacramental (Baptism, Confirmation, etc.) information was recorded under your maiden name. Having this on file will expedite future requests for information by yourself or your children.

  • We need you to indicate gender because several given names can often be used for either a male or female child. We don't want to code your child (or yourself!) incorrectly.

  • Yes, if your sibling is not a member of the Parish (meaning she did not complete her own Census form), list her as an adult on your form.

  • If your permanent residence Is here in Granby or the surrounding area, a vacation home would be considered an “Alternate Address." Even if you don't have a permanent vacation address, we'd still like to know what months you typically are away for mailing purposes.

  • YES! If you are a parishioner, but don't want envelopes, you should still complete a Census form. However, we strongly encourage you to use contribution envelopes for whatever amount is an honest return to God. Year-end contribution statements issues for tax purposes are generated from envelopes (the only way contributions are recorded).
